Purpose:
To help you design a functional SMART Goal step-by-step that you can use for personal and professional achievement. This SMART Goal Setting Worksheet is also suitable for use in the workplace.

Instructions
- Download and print this SMART Goal-Setting Worksheet by clicking on the download button above. The file is in Adobe PDF format.
- Start by stating your SMART goal in the present tense as if you have already achieved it. Associate a date with your SMART goal. An example of this would be, "The date is March 1, 2010 and I feel great at 138."
- It's important to always state your goal in the positive and not in the negative because your reticular activating system only focuses on the main subject of your sentence. For example, if I said, "Don't think about pink elephants," what is the first thing that pops into your head.....PINK ELEPHANTS. It's better to state a specific request such as, "Visualize a white cat."
- Next, you will need to create a measuring stick for your SMART goal. A measuring stick is a way to measure the progress and success of your goal. There is a saying that goes, "If you can't measure it, you can't manage it." This is true of goals. The best way to create a measuring stick for your goal is to use something that is quantifyable, such as numbers, weights, money, etc. You want to make the measuring system as objective as possible so that any stranger can come along and look at your results and tell you if you have successfully achieved your goal or not.
- Next, list the milestones for your SMART goal. You can think of milestones as mini-goals within your goal. Milestones help keep you on track towards the completion of your goal.
- Next, list the specific action items necessary to complete your goal. Actions items are defined as tasks that can be done in one "sitting."
- Any goal worth achieving is also worth celebrating. In the space provided, write out how you will celebrate the achievement of your goal.
- The number one reason why most people fail at achieving their goal is because of a lack of accountability. That's where having an accountability partner handy is useful to keep you on track. Use the space provided to list someone that you trust who is responsible enough to keep you on track with your goal. Often it helps if your accountability partner also shares the same goal.
- Finally in the space at the bottom, commit yourself to your goal by signing your goal form.
- Keep your goal form visible each day as a contant reminder of what you must do to achieve your goal.
