Purpose: To assess the taker's level of balance in their life. This assessment will give feedback on the areas of life that are doing well as well as those that are lacking.






Instructions


  1. Download and print the form by clicking on the download button above. The file is in Adobe PDF format.
  2. Notice that the wheel is sectioned off into eight segments and each segment has concentric circles numbered from 1 to 10.
  3. For each of the eight segments, determine subjectively how satisfied you are with that particular area of your life. Use the guide below the circle to help you select a number.
  4. Using a pen or pencil, shade in the segments corresponding to the level of each life success value that you are experiencing in your life right now.
  5. Examine your results. Ideally, your "wheel" should be perfectly round and large in size. If it is not round in shape, then you will be able to identify the areas in your life that are lacking.
  6. You may take this assessment periodically throughout the year as your life balance constantly fluctuates over time.
  7. Coaches and mentors can use this assessment with their clients to gain a better understanding of the areas that require more attention and as a starting point for further conversation.
